This is a physician-owned, four-location dermatology and aesthetics practice serving the greater Atlanta area, offering comprehensive medical dermatology, Mohs surgery, and a full-service aesthetics division. The practice is independently owned with no private equity involvement, and continues to expand its clinical service lines and footprint.

This is an excellent opportunity for an experienced healthcare operations executive to lead a well-established practice with direct access to physician ownership and genuine authority over how the organization runs.

The Opportunity

The Chief Operating Officer is the senior-most administrative executive in the organization, reporting directly to the two physician Owners. The COO is accountable for the operational and financial performance of all four clinics and the aesthetics division, leads the practice’s management team, and partners with the Owners on strategic planning, growth, and major business decisions. The COO carries direct accountability for revenue growth and EBITDA performance across the practice.

The role is scoped deliberately. The COO is expected to lead through the management team and hold vendors accountable rather than absorb work personally, and to build an operating structure that does not depend on any single person. The practice operates without a CEO layer, which means real authority, real exposure, and a short line to decisions.

The right candidate is someone energized by building operating discipline, developing managers into stronger leaders, and bringing clarity to a growing organization that has significant room to execute.

Key Responsibilities

Executive and Strategic Leadership

  • Partner with the physician Owners on strategic planning, growth initiatives, service line expansion, and major capital and business decisions
  • Translate ownership objectives into annual operating plans with named owners, deadlines, and measurable outcomes
  • Serve as the point of resolution for cross-departmental priorities, escalations, and resource decisions
  • Return the Owners’ time to clinical care and strategic direction by taking genuine ownership of operations

Financial Leadership

  • Own the operating budget, forecast, and expense discipline across all locations and the aesthetics division
  • Drive EBITDA growth and margin improvement, with accountability for both revenue performance and the cost structure that supports it
  • Establish a disciplined monthly close and financial review rhythm, working with the practice’s accounting resources and external CPA firm
  • Help define and build out the internal finance function, then lead through it
  • Deliver location-level profit and loss visibility, including appropriate separation of administrative overhead from operating locations
  • Monitor and improve revenue cycle performance in collaboration with the Billing Manager
  • Own payer relationships and lead the renegotiation of managed care contracts, including reimbursement terms for advanced practice providers
  • Understand expense and payroll growth at the line-item level and hold cost growth in a defensible relationship to revenue growth
  • Oversee payroll, compensation structure, and performance-based incentive programs

Operations and Team Leadership

  • Lead a management team spanning HR, finance, billing, call center, Mohs, dermatopathology, aesthetics, and site management across four locations
  • Oversee the recently launched dermatopathology service line and lead the development of an in-house laboratory
  • Chair the weekly management team meeting and maintain a consistent one-on-one cadence with all direct reports
  • Maintain regular on-site presence across locations to ensure consistent standards, staffing, and patient experience
  • Coordinate provider schedules across sites to maximize patient access
  • Build and maintain an operating dashboard covering access, provider productivity, revenue cycle, patient experience, staffing and turnover, aesthetics performance, and progress against strategic initiatives

Aesthetics Division

  • Own the operational and financial performance of the aesthetics division, including service and product mix, pricing, provider productivity, inventory, and reconciliation
  • Bring or develop genuine commercial depth in aesthetics as a growth line rather than as an adjunct to medical dermatology

People and Culture

  • Direct hiring, performance review cycles, and personnel decisions in partnership with HR
  • Manage the PEO relationship, including annual benefits selection and renewal
  • Develop managers into stronger leaders through coaching, clear expectations, and accountability
  • Build staffing pipelines for clinical and administrative roles, with candidate pools for high-turnover positions and succession plans for key roles, so vacancies are filled from a bench rather than from a standing start
  • Own retention as a measured outcome, particularly in clinical support roles

Vendors, Technology and Facilities

  • Own vendor selection, contract review and negotiation, and renewal management across IT, marketing, telecommunications, software, and clinical supply
  • Oversee the managed IT services relationship and the practice management and EMR platform. This role directs and holds vendors accountable; it does not perform hands-on technical work
  • Manage facilities and lease matters, including negotiations, amendments, and tenant improvement projects, with an eye toward potential practice acquisition of current or future office space
  • Oversee inventory and procurement systems across all sites

Compliance and Clinical Operations Support

  • Coordinate with the Compliance Officer to maintain regulatory readiness across all locations
  • Build the operational infrastructure for new clinical service lines as the practice expands
  • Support clinical leadership on provider workflow, capacity, and coverage planning

  • Delegating deliberately. Recognizing when work should not sit with the COO, and placing it correctly, is a core competency of this role

What Success Looks Like in the First 12 Months

  • A documented responsibility map is in place, with work positioned where it belongs rather than concentrated by default
  • The management team operates with clear expectations, a consistent cadence, and defined decision rights
  • Monthly financial close and review run on a predictable rhythm, with location-level reporting the Owners trust
  • Expense and payroll growth are understood at the line-item level and managed against plan
  • EBITDA is measured and reported with a clear improvement plan agreed with the Owners
  • Managed care contracts have been reviewed and a renegotiation plan is underway
  • Provider schedules stay full and patient access improves across all four locations
  • The aesthetics division has clear performance measures and reconciliation that closes on time
  • Vendor contracts and renewals are actively managed with no unplanned auto-renewals or lapses
  • Retention in clinical support roles is measured, understood, and improving
  • The Owners spend their time on clinical care and strategic direction, not daily operations

Qualifications

  • Minimum 5 years of progressive operations leadership in multi-site healthcare, with direct profit and loss accountability
  • Demonstrated success leading managers across multiple locations and functions
  • Strong financial acumen, including budgeting, forecasting, variance analysis and financial reporting, with the ability to interrogate the numbers and hold the finance function to account
  • Demonstrated track record of improving EBITDA or operating margin across a multi-site organization
  • Experience negotiating and executing vendor contracts, leases, and service agreements
  • Working fluency across HR, IT, compliance, and facilities functions, sufficient to direct specialists and vendors
  • EMR or practice management platform experience; ModMed (EMA) preferred
  • A self-starter who can identify issues, exercise sound judgment, and take initiative
  • Master’s degree preferred; qualified candidates without one are encouraged to apply
  • Dermatology, aesthetics, or specialty physician practice experience strongly preferred
  • Experience in physician-owned private practice, particularly reporting directly to owners without a CEO layer, preferred
  • Experience building or maturing a finance and reporting function, rather than only inheriting one, preferred
  • Commercial experience in aesthetics, medical spa, or retail-adjacent healthcare services preferred
  • Experience negotiating or renegotiating managed care and payer contracts preferred
  • Experience with commercial lease negotiation or property acquisition preferred
  • Experience launching or scaling ancillary service lines, such as pathology or laboratory services, preferred

Compensation and Work Environment

  • Base salary starting at $200,000, commensurate with experience, with a performance-based incentive component
  • Full-time and on-site, with regular presence required across all Atlanta-area locations
